It is important that both you and your partner acknowledge that you have reviewed all of the information on this Marriage Application, including, but not limited to, names, dates of birth, address, Social Security number, and marriage number. The couple confirms and agrees that if discrepancies are found after the marriage license has been recorded, they will be required to pay an amended license fee to the County Clerk.
 

A court order and associated filing fees may apply and will be required.  You may need to retain an attorney for an additional fee (if necessary) if the Court Clerk deems it necessary. By clicking you both accept as a couple any additional fees applicable for any discrepancies.
